Our non-profit organization, which provides top-quality care for 200 in-patients and several thousand out-patients, has initiated a 24/7 call center to provide:
• Timely responses to daily challenges experienced by caregivers/family members living in the community
• First contact with a specialized nurse/social worker trained in Alzheimer patient care
• If necessary, further contact for each call is directed to other professionals including geriatricians, psychiatrists, physiotherapists, dieticians, activity facilitators and expressive arts therapists.
• Medical records for each patient/family are written and stored in the system according to the needs of each referral and as such follow-up
